Debridement

This is where we carefully remove dead or infected tissue from a wound. It helps create a clean base for healing to start.

Sometimes wounds are too big to heal on their own. In these cases, we can take healthy skin from another part of your body and place it over the wound to speed up healing and lower the chance of infection.

Surgical offloading to support wound healing

Why is debridement necessary?

When a wound contains dead or infected tissue, it slows or blocks the natural healing process. The damaged tissue can also harbour bacteria, leading to deeper infections that threaten the entire limb.

Debridement removes this unhealthy tissue, either surgically or through specialised wound care techniques or dressings, creating a healthy environment for new skin and blood vessels to grow. Regular debridement can be a key part of saving a limb that would otherwise be at risk.

What is Charcot foot?

Charcot foot is a condition where the bones, joints and soft tissues of the foot weaken and collapse, often without much pain. It usually develops in people with diabetes who have nerve damage (neuropathy), meaning they cannot feel injuries happening.

Left untreated, Charcot foot can lead to major deformities, unstable walking and high risk of wounds or infections. Early stages may be treated with special boots or casts to protect and stabilise the foot. In more advanced cases, surgery is needed to realign bones, stabilise joints and prevent permanent disability.

Can't you just remove the blockage?

In some situations it may be possible to remove the fatty deposits that cause narrowing of the the blood vessels. This procedure (called an endaterectomy) involves opening the blood vessel and peeling away the diseased fatty deposit. The vessel is then repaired using stitches. Whilst this operation can dramatically improve blood flow in some patients a general anaesthetic is usually required and full recovery can take a few weeks. Similar to bypass surgery, there are minimally invasive options.

Atherectomy is a keyhole procedure where some blockages can be removed without making a big cut in your leg and may be more appropriate if you have other health problems that mean a general anaesthetic is more risky. Sometimes the blockage may be very hard like concrete or bone. In this situation a special keyhole technique called lithotripsy can be used to fracture the blockage using high frequency sound waves before stretching it with a balloon (angioplasty).
